Attached is a personal progress guide for record purposes only to download and save onto your computer.Personal Progress Guide

For each day please record your calorie intake and actual weight by replacing the sample values already in place. The charts contained in the document will change accordingly to show your progress. You will need Microsoft Excelâ„¢ or a compatible spreadsheet program to use this guide.

Please note that the figures on the guide are based on a patient record but not indicative of ideal calorie intake or actual weight.
